• Acord 125, 126 and/or 130

• 5 years loss history (currently valued)

• Experience modification worksheet (NCCI or other applicable rating agency

• Detailed narrative of operations

• Contractors Supplemental Application for general liability

• Workers' compensation supplemental for trucking or staffing

• Owner resume (for new company or no prior coverage)

Minimum Requirements

• $ 25,000  minimum premium required for general classes

• $ 50,000  minimum premium required for construction and trucking class codes

• $100,000 minimum premium for roofing and tree trimming class codes

• $100,000 minimum premium and a minimum $10,000 retention (may be higher) for staffing companies

Exclusions

• Flat bed trucking risks

• Hot tar roofing risks

• Underground operations

• General contractors or any contractors employing greater than 10% subcontractors
